Enfin, Louis XIV, affecta Bicêtre aux malades civils, et ordonna, en 1670, la fondation d'un hôtel royal "pour le logement, subsistance et entretenement de tous les pauvres officiers et soldats de nos troupes qui ont été ou seront estropiés, ou qui, ayant vieillis dans le service icelles, ne seront plus capables de nous en rendre". La même année, le 30 novembre, les travaux débutèrent sur les plans de Libéral Bruant. Jules-Hardouin Mansart fut chargé d'agrandir l'église du centre de laquelle s'élève le dôme.
