Spécimen, sans filigrane. Un trou, centre-haut gauche. Imprimé en bleu, fond rose. Deux signatures : "Le Président" et "Le Trésorier". Sans série. Sans numéro. IMP. THEVENOT_ST DIZIER en bas au centre / Tampon "ANNULÉ" dans un rectangle (16 x 43 mm) noir, transversal, en haut à gauche. Imprimé en marron sur fond blanc. Remboursement : 11 novembre 1920.
